Probably because this isn’t a normal version of Sylvana’s curse. It is a highly decayed version of it that’s been sitting in Alistair’s lair for hundreds if not thousands of years. Who knows what all that time has done to the virus? Not to mention, the virus was never meant to interact with someone’s blood stream or be in their bodies outside of an orb. So it’s not only decayed and corrupted, but concentrated too.
It shouldn’t be too surprising that it’s not acting like the normal gender curse and is only turning people into sexualized women.
